Insurance
If you have a Canadian-based extended health care plan, Tall Tree will direct bill if possible, or alternatively will provide you with receipts that you can submit for reimbursement.
If you have a US-based health insurance plan, Tall Tree will provide you with receipts that contain the information you will need to seek reimbursement. You will be responsible for submitting this information. I recommend that you confirm directly with your health insurer carrier the details of your particular plan and your projected financial responsibilities in advance of your first appointment. You may reference Tall Tree Integrated Health Centre and my NPI number (1578954376) to obtain confirmation from your insurance company about your benefits.
